  • The Data Subject hereby honestly declares that he/she gives his/her explicit and unconditional consent to the Controller to process his/her personal data to the extent indicated above, for the above purpose and during the above period.

  • The data subject has the right to withdraw his/her consent at any time. Withdrawal of consent shall not affect the lawfulness of processing based on consent prior to its withdrawal.

  • The rights of the data subject are defined in particular in Sections 19 to 28 of the Act. The data subject is obliged to provide true and up-to-date personal data pursuant to Section 9 of the Act.

  • The Data Subject confirms that the Operator has fulfilled the notification obligation in accordance with Section 19 of the Act.
